Required for all patients who have never been seen in our practice - Including those scheduling for acute or sick concerns.  

Includes Comprehensive evaluation, medical history review, and personalized treatment plan. 

For ongoing care, chronic condition management, medication management, and routine follow-ups.

 Evaluation and treatment of acute, non-emergency medical concerns, such as:

• Cold, flu, cough, or sinus symptoms
• Sore throat or ear pain
• Urinary symptoms
• Minor infections

 Convenient virtual visits for follow-ups, medication refills, and select acute concerns when clinically appropriate.
